- Head north on Ashley Drive toward I-275 N — you’ll pass by the Tampa Museum of Art and University of Tampa on your left.
- Merge onto I-275 N via the ramp to I-75.
- After about 10 minutes, take exit 59 for I-75 N toward Ocala.
- Stay on I-75 N for about 25 miles — you’ll pass University Mall and the Wesley Chapel area.
- Take Exit 285 for SR-52 toward San Antonio/Dade City.
- Turn right onto SR-52 E (State Road 52) and drive about 3 miles.
- You’ll see Mirada Lagoon signage and Publix at Mirada nearby.

- Turn left onto Mirada Blvd.

Starting Point: Clearwater Beach Pier 60
- Head east on Court Street toward US-19 — passing Clearwater City Hall and the Pinellas County Courthouse.
- Merge onto US-19 N briefly, then take the ramp for FL-60 E / Gulf-to-Bay Blvd.
- Continue east on FL-60 for about 10 miles — pass the Clearwater Mall and cross the Courtney Campbell Causeway into Tampa.
- Merge onto I-275 N, then quickly transition to I-75 N toward Ocala.
- Stay on I-75 N for about 20 miles, passing by the Tampa Premium Outlets and Wesley Chapel area.
- Take Exit 285 for State Road 52.
- Turn right (east) onto SR-52, go about 3 miles.
- Look for signs for Mirada Lagoon and Publix at Mirada.
- Turn left onto Mirada Blvd.

Qualifying conditions include chronic pain, PTSD, cancer, epilepsy, glaucoma, anxiety, depression, and more. Dr. Kachi will assess your symptoms during the evaluation.
Cards are valid for 1 year from the date of issue. Follow-up appointments with your physician are required every 7 months per state regulations.
Both new evaluations and renewals may be available via telehealth, depending on your case. Call us to confirm eligibility.
Royal CarePlus offers transparent and affordable pricing. Evaluation fees start at competitive rates, and the state MMUR card costs $75 annually.
